Don't worry! Our paperwork doesn't take too long to complete. Filling out your paperwork ahead of time and bringing it with you will help to ensure that you are seen right on schedule.

New and Returning Patients.

​We are updating all patient files, so please be sure to bring the first 4 forms and read through the Notice of Privacy Practices, even if you have seen Dr. Cochrane before at any time in the past.
